Salt Lake Masonic Temple, operated by Masonic Temple Association, offers SLMT Notifications only to authorized staff and administrators who choose recurring automated operational and security SMS alerts. Initial enrollment is limited to two consenting staff recipients. This program does not send marketing, fundraising or public rental inquiry messages.
How staff provide consent
The office provides the separate optional paper form during staff SMS enrollment. A staff member enters their mobile number, checks the SMS consent box, and signs and dates the form. The office verifies the staff authorization and signed record before enrollment. Leaving the box unchecked does not enroll anyone. The office retains completed forms privately. This page shows a blank form for review and printing; it does not collect online SMS enrollment.
Alerts may concern door access, alarms, HVAC and other building systems, integration failures and service restorations. Message frequency varies and multiple alerts may occur in a day. Message and data rates may apply. Consent is voluntary and is not a condition of purchase. Reply STOP to opt out or HELP for assistance. Mobile information and SMS consent are not sold or shared with third parties or affiliates for marketing or promotional purposes.

Confirmation and choices
After service activation, each approved enrollment receives this confirmation before recurring alerts:

STOP ends enrollment and cancels pending alerts. START lifts a carrier block but does not enroll an unknown number or override the office’s consent review. Re-enrollment requires renewed signed paper consent and staff authorization. Public contact forms and the separate PrivateEvents.co rental form do not enroll recipients in this staff program.
